Collaborative Communication is Critical

Collaborative healthcare for Care Teams is a critical element of successful patient outcomes at any time, but it’s even more important during the COVID pandemic. Of the many lessons learned about patient care during this extraordinary time, collaborative, aligned communication among Care Teams is a vital one. Mark Zarefsky, contributing writer for the AMA, states: “Ineffective communication omits key elements of a patient’s story or is presented in a way that is not simple to understand. The effect of this can be catastrophic… it is reported that data from 2010-2013 shows that ineffective communication is one of the top three root causes of sentinel events.” 

One of the best ways to align communication is to implement HCI’s eSitter – live video streams of patients for close observation and monitoring for safety issues and fall risks.

Part of effective care team collaboration has to be a harmony between a patient’s emotional and physiological care needs, technology to effectively support those needs, clinical requirements and regulatory/administrative rules to address patient care. 

Highly Connected Care Teams are More Effective

The American Society of Geriatric Care has released a study assessing “the impact of team structure composition and degree of collaboration among various providers on process and outcomes of primary care,”the results of which show that highly connected care teams with solid collaboration in place present fewer overall hospitalizations, lower costs, lower incidences of ER admission and “more coordinated care of older patients.” The study looks at team leadership and how a less centralized model can deliver these results. Automation for rounding, dissemination of patient information and alert response is a touch away with interactive whiteboards and tablets. In turn, everyone has the information they need, when they need it, and can update the team immediately, with accurate visual cues pulled directly from the EHR across the enterprise from integrated systems in the facility.
